Reflex syncope in children and adolescents.

چکیده
Syncope can be defined as a temporary loss of consciousness and postural tone secondary to a lack of adequate cerebral blood perfusion. The incidence of syncope coming to medical attention appears to be clearly increased in two age groups—that is, in the young and in the old (fig 1). An incidence peak occurs around the age of 15 years, with females having more than twice the incidence of males. 2 Syncope is an infrequent occurrence in adults. The incidence of syncope progressively increases over the age of about 40 years to become high in the older age groups. A lower peak occurs in older infants and toddlers, most commonly referred to as ‘‘breath-holding spells’’. The incidence of syncope in young subjects coming to medical attention varies from approximately 0.5 to 3 cases per 1000 (0.05–0.3%). Syncopal events which do not reach medical attention occur much more frequently. In fact, the recently published results of a survey of students averaging 20 years of age demonstrated that about 20% of males and 50% of females report to have experienced at least one syncopal episode. By comparison, the prevalence of seizures in a similar age group is about 5 per 1000 (0.5%) and cardiac syncope (that is, cardiac arrhythmias or structural heart disease) is even far less common. By far the most common cause of syncope in young subjects is a reflex syncopal event and in particular a vasovagal faint This disorder is the focus of the present review.

متن کاملDysfunctional Behaviors in Children and Adolescents with Neurocardiogenic Syncope
Objective: We evaluate a large sample of children and adolescent with suspected neurocardiogenic syncope (NCS) and we compared the results with a healthy sample, with the aim to reveal a possible correlation between syncopal events and dysfunctional behaviors. Methods: A total number of ninety two patients with NCS (median age 14.7 years) were evaluated with Head-Up Tilt Test (HUTT) and psychol...
متن کاملIndividualized Treatment of Syncope in Children: State-of-the-Art
Syncope is caused by transient cerebral ischemia, for various reasons, leading to the loss of consciousness and muscle tension maintenance, resulting fainting of the patients. It is a common emergency for children and adolescents. A statistical analysis shows that about 15–25% of children and adolescents had suffered syncope at least once. Suffering syncope has greatly impacts on physical and m...
متن کاملCardiac pacing for severe childhood neurally mediated syncope with reflex anoxic seizures.
OBJECTIVE To determine whether permanent cardiac pacing could prevent syncope and seizures in children with frequent severe neurally mediated syncope, and if so whether dual chamber pacing was superior to single chamber ventricular pacing. METHODS Dual chamber pacemakers were implanted into 12 children (eight male, four female) aged 2-14 years (median 2.8 years) with frequent episodes of refl...
متن کاملBaseline heart rate variability in children and adolescents with vasovagal syncope
PURPOSE This study aimed to evaluate the autonomic imbalance in syncope by comparing the baseline heart rate variability (HRV) between healthy children and those with vasovagal syncope. METHODS To characterize the autonomic profile in children experiencing vasovagal syncope, we evaluated the HRV of 23 patients aged 7-18 years and 20 healthy children. These children were divided into preadoles...
